چکیده

prise about 2% of all fractures, but the incidence is increasing due to increasing numbers of high-speed vehicular crashes and suicide attempts. Mortality associated with isolated pelvic injury, independent of severity, has been reported to be low: 1%–2%. Among multiply injured victims of blunt trauma, however, almost 20% have injuries to the pelvic ring. In this group, when closed pelvic ring disruption is associated with multiple injuries, the mortality rate rises to 10%–15%. Pelvic fractures associated with intra-cranial mass lesions or notable abdominal injuries have mortality rates as high as 50%. The mortality associated with open pelvic fractures has been shown to be 30%– 50%. The parameters that predict mortality are age, injury severity score (ISS) and the existence of severe hemorrhage. Exsanguinating hemorrhage is the major cause of death in the first 24 hours after trauma. Immediate recognition of hemorrhagic shock and effective control of bleeding must be pivotal in every resuscitation effort. Appropriate recognition and management of serious pelvic fractures is also integral to resuscitative strategy. Management of these potentially lethal injuries requires expedited stabilization by a multidisciplinary team of trained personnel with a defined treatment protocol. Multidisciplinary clinical-pathway and coordinated joint decision-making improves patient survival. A postmortem study by Wright and colleagues has shown that the average ISS of patients dying from pelvic fractures was much higher in patients treated by a protocol of care than in those treated with a “non-system” approach on an ad-hoc basis. As the understanding of these potentially fatal injuries improves, priorities of early management of pelvic ring injuries are evolving. This review article summarizes the current trend in emergent management of pelvic fractures, based on available evidence. A protocol designed to facilitate organized and systematic care of the serious pelvic fracture in multi-system trauma is proposed.

The functional outcome of surgically treated unstable pelvic ring fractures by open reduction, internal fixation

Abstract Background: Unstable Pelvic fracture, a result of high energy antero-posterior compression injury, has been managed based on internal fixation and open reduction. The mode of fixation in Unstable Pelvic fracture has, however, been a subject of controversy and some authors have proposed a need to address the issue of partial breach of the pelvic ring elements in these injuries. This st...

Emergent stabilization of pelvic ring injuries by controlled circumferential compression: a clinical trial.

BACKGROUND Pelvic ring injuries are associated with a high incidence of mortality mainly due to retroperitoneal hemorrhage. Early stabilization is an integral part of hemorrhage control. Temporary stabilization can be provided by a pelvic sheet, sling, or an inflatable garment. However, these devices lack control of the applied circumferential compression. We evaluated a pelvic circumferential ...

Hemodynamically unstable pelvic fracture management by advanced trauma life support guidelines results in high mortality.

The purpose of this study was to examine the acute outcomes and mortality rates of an Advanced Trauma Life Support guideline approach for managing hemodynamically unstable pelvic ring injuries. We retrospectively reviewed the acute outcomes of 48 consecutive patients with hemodynamically unstable pelvic fractures. Patients underwent treatment via the advanced trauma life support protocol, with ...

Early Postoperative Displacement of Combined Pelvic Ring Injury With Acetabular Fracture.

Combined pelvic ring and acetabular injuries present a management challenge. The literature on this topic is scarce, with few outcomes studies available. This retrospective study assessed whether the incidence of postoperative displacement and loss of reduction is higher with combined injuries compared with isolated pelvic ring injuries and isolated acetabular fractures. The charts and radiogra...

History of pelvic fracture management: a review

High-energy pelvic fractures represent potentially life-threatening injuries due to the risk of acute exsanguinating retroperitoneal hemorrhage. The first report of a severe pelvic ring disruption dates back to Charles Hewitt Moore's seminal publication from 1851. Significant advantages in the understanding of injury mechanisms and treatment concepts of pelvic ring injuries evolved in the 20(th...
